This is an extremely tricky situation! And you are in a particularly uncomfortable position.
First of all, it sounds like this woman is in dire need of psychiatric evaluation and treatment. (Do you live in the US?) The problem is, depending on state laws, it is extremely difficult to enforce treatment on anyone who truly does not want it. In some states, if there is a genuine threat of this woman hurting herself and/or someone else, she can be committed for hospitalization and treatment by anyone.
In some states/cities, there are psychiatric mobile crisis units that anyone can call and request an evaluation for a person exhibiting suicidal/homicidal behavior. You really need to check with an attorney to see what your state law requires.
Another possibility is calling your local Department of Social Services to have them investigate the welfare of the children.
